The reasons for failure include insufficient creepage distance and flawed design in some products, leading to breakdown during the withstand voltage test. Once a breakdown occurs between the primary and secondary circuits, the voltage (220V) from the primary circuit will pass through the breakdown point to the secondary circuit, potentially causing electric shock, battery explosion, or fire.
